This procedure facilitates immediate impressions for crown and bridge procedures. Redundant or hyperplastic tissue can be removed within a bloodless field, with minimal - if any-post-operative pain. The outcome increases the length of the clinical crown for improved restoration retention and esthetic harmony.
Advantages
Compared to traditional surgical techniques, this procedure can be done without scalpel or sutures, establishing ideal tissue architecture without post-operative shrinkage. Final impressions for restorations can be completed in the same appointment, leading to predictable results time after time.
Technique and Laser Settings
The SoftLase Pro™ or OrthoLase™ should be set at 1.0 - 1.4 Watts in a continuous mode of operation. The fiber must be initiated in accordance with the prescribed protocol, followed by administration of local anesthetic. The site must be probed and sounded to assure an adequate zone of attached gingiva. Use the side of the fiber with a light paint brush stroke to establish an external 45° bevel when excising the gingival margin, in order to achieve the desired tissue architecture. A probe can be used to create bleeding points to serve as a guideline for laser incision. Care should be given to assure an adequate zone of attached tissue and not leave a 90° butt shoulder margin which will affect esthetics and cause plaque accumulation. If decay or other defects are found within 2mm of the base of the periodontal pocket, osseous surgery will most likely be required. Post-operative analgesics are generally not needed for soft tissue crown lengthening and gingivectomy with the SoftLase Pro™ or OrthoLase™.
